Use the form below to submit a copy of your dealer's FFL document in its entirety. You will need to contact you dealer and request this scanned document, or have you dealer complete this form including your name and order number.
Any submissions lacking any of this information, will cause delays with your order. Please ensure you are submitting the correct documents to avoid delays.
